in order to schedule an appointment.
There are two options to request an appointment:
1) In person
2) Email
Please provide required documents in person:

An appointment will be scheduled at that time based on availability.
OR
Please send required documents and phone number via email:

Documents will be reviewed and an appointment will be scheduled
based on availability via phone call.
For gasoline assistance, you must apply at Eblen Charities in person.
Requires proof of medical appointment, job interview or new hire.
YOU MUST BRING AN ID to obtain a gas card and
it will be required to present at gas station.
(MUST be within 24 hours prior to scheduled appointment and
no later than end of same day of appointment with proof of completion).
